Buffalo, NY – On Monday, November 2021 The Greater Buffalo United Community Based Organization (GBUCBO) gave 1000 turkeys and sides to GBUAHN members, Urban Family Practice patients, and community partners at its 1195 Main Street location
For the past few years, Dr. Raul Vazquez has ensured safe and happy Thanksgivings for the families in need through the Greater Buffalo United Accountable Healthcare Network (GBUAHN). Since the creation of GBUCBO, the reach into the community can be greater. The care coordination staff from GBUAHN have carefully chosen Health Home members with a need to deliver a basket to them. Understanding and providing solutions to social determinants of health is a daily part of member care.
The same effort is being done from Urban Family Practice (UFP) and GBUCBO. UFP has identified families of the practice that can benefit from a Thanksgiving dinner. GBUCBO staff has reached out to community partners that have direct connections to families in need.
“Creating GBUCBO allows me to do more,” said GBUCBO President, Dr. Raul Vazquez. “I remember what it was like to go without a meal, and I want eliminate that feeling from as many families as possible during the holiday season.”The mission of the Greater Buffalo United Community Based Organization (GBUCBO) is to transform healthcare by removing barriers created by social determinants of health in underserved communities.
GBUCBO is an affiliate of G-Health Enterprises, a health care organization that encompasses several affiliates: the Greater Buffalo United Accountable Healthcare Network (GBUAHN), the Greater Buffalo United Accountable Care Organization (GBUACO), and Urban Family Practice (UFP), the Greater Buffalo United IPA (GBUIPA) and the Greater Buffalo United Community Based Organization (GBUCBO).
