Oishei Healthy Kids Health Home is seeking candidates to become part of a dynamic team providing care coordination services for children, adolescents and families in the Western New York area. The members enrolled in the program have chronic behavioral and/or medical conditions.  Openings are available for the following:

Community Health Workers are responsible for community outreach & engagement, education and enrollment of new members to the Health Home. 

·  High School Diploma/GED and 2 years of experience working with pediatric/adolescent populations in a health/social services field preferred.           

·  Valid NYS driver’s license and reliable transportation required.   Positions start at $16/hour and a good benefit package. 

Care Managers are responsible for the coordination of child/adolescent care needs.  They conduct a comprehensive health assessment and CANS- NY assessment. They assist the child, adolescent & families to access care in the most appropriate setting. They also conduct interdisciplinary case reviews and set goals for the members.  

·   Bachelor’s degree in Community/ Public Health, Social Work. Psychology or related field and 2 years of experience working with pediatric/adolescent populations in a behavioral health or medical setting required. 

·   Valid NYS driver’s license and reliable transportation required.  CANS-NY certification required within 30 days of hire.  Care Managers starting rate is $20/hour, mileage reimbursement and good benefit package.
