CARE COORDINATOR – Bilingual-Spanish/English Speaking – Openings in Buffalo and Jamestown! Up to $2000 signing Bonus. The Bilingual Care Coordinator provides the activities of case management to improve health outcomes for patients and improve disease-related care to chronic conditions.

Required education: Bachelors (plus 1 years experience: OR Associate’s ( plus 2 year’s experience)

RECEPTIONIST – Bilingual – Spanish/English – The Bilingual Receptionist provides front desk reception, professional customer service, record-keeping  and other administrative support to Evergreen Health’s Centers for Care Coordination.

Required education: Associate’s (plus 1 year experience) OR High School Diploma/GED Equivalency (plus 2 years’s experience)
