Help Wanted -Health Nurse Educator
(Dunkirk, NY Area)
Complete health screenings/assessments on customers; develop a health plan; make referrals to health care providers based on assessment and customer need, follow up, and provide holistic health education. Holistic health education will promote positive health outcomes, reduce costs, and empower customers to make informed, proactive decisions about their health care. Position is responsible for planning, developing and implementing holistic health education for customers. Education to include, but not limited to : The health system and proper use of it; Importance of a medical home; Health Insurance and associated costs (co-pays, premiums, prescriptions); High Risk Behaviors; Being proactive vs. reactive behaviors; and Importance of routine health screenings.
